Privacy Policy

This policy covers gatewayfamilydentistry.com (the “Site”), operated by Gateway Family Dentistry in Chandler, Arizona. It explains what information the Site collects, how we use it, and the choices you have.

Health information you share with us as a patient is protected under federal law (HIPAA) and our Notice of Privacy Practices, which is available at our office. This Site is not designed to receive health records. Please do not include detailed medical information in a website form.

When you request an appointment or contact us through the Site, we collect what you enter: your name, email address, phone number, whether you are a new or returning patient, the reason for your visit, your message, and whether you give us permission to call you. We keep your IP address and browser details with the submission to detect spam.

When you opt in to our text messaging program, we collect your mobile number and your consent. Our SMS Policy covers that program in full.

Like most websites, the Site records technical details about visits: IP address, browser and device type, pages viewed, time on page, the site that referred you, and approximate location derived from your IP address. We use these services to collect it:

• Google Analytics, which sets cookies to distinguish visitors and measure traffic.
• PostHog, which stores a visitor identifier in cookies or browser storage to measure how pages and forms are used. Session recording is turned off.

Some pages embed content from other companies: Google Maps for directions, YouTube for videos (a preview image loads from YouTube; the player itself loads in privacy-enhanced mode only after you press play), and a patient reviews widget. When those embeds load, the company providing them may set its own cookies and collect its own usage data under its own privacy policy.

• To respond to your request and schedule or confirm appointments.
• To send text messages you have opted in to receive.
• To run, secure, and improve the Site, including detecting spam submissions.
• To understand which pages and campaigns bring patients to the practice.
• To meet legal, regulatory, and record-keeping obligations.

We do not sell your personal information. We share it only in these cases:

• With service providers that process it on our behalf: website hosting and storage (Cloudflare), email delivery for form submissions (Resend), a backup copy of form submissions (Google Forms), and the analytics services listed above. These providers may use your information only to deliver their service to us.
• When required by law, subpoena, or a request from a government authority.
• To protect the rights, safety, or property of our patients, our staff, or the practice.
• As part of a sale, merger, or transfer of the practice, subject to this policy.

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging opt-in data and consent will not be shared with any third party. For the text messaging program, the sharing terms in our SMS Policy govern.

If you opt in, we may text you with appointment reminders and scheduling, follow-up care, billing updates, and marketing. Message frequency varies. Message and data rates may apply. Reply STOP to any message to opt out, or HELP for assistance. Consent to receive texts is not a condition of receiving care. The full program terms are in our SMS Policy.

You can block or delete cookies in your browser settings. The Site will still work, but blocking cookies does not necessarily stop analytics collection because some services also use browser storage. Google offers a browser add-on that lets you opt out of Google Analytics; it does not affect PostHog.

You can ask us to correct or delete information you submitted through the Site by contacting us at the address below. Requests about your dental records are handled under our Notice of Privacy Practices, not this policy.

We keep form submissions for as long as we need them to respond to you and maintain practice records, and for as long as the law requires. Analytics data is kept under each provider’s default retention settings.

The Site is served over HTTPS, and access to form submissions is limited to staff who need it. No method of transmission or storage is completely secure, so we cannot promise that unauthorized access will never occur.

The Site is not directed at children under 13, and we do not knowingly collect information from them through it. Parents and guardians should submit requests on a child’s behalf.

The Site links to social networks and review sites. Those sites have their own privacy policies, and we are not responsible for how they handle your information.

We may update this policy from time to time. The date at the top shows when it last changed. Continued use of the Site after a change means you accept the updated policy.
